If you don't have a printer, you can simply copy the required information (below) on a plain sheet of paper and fax it to us at the above listed number.

To avoid delays in processing your application, use dark ink and write very legibly. Upon receiving your fax, we'll begin processing it immediately.

If you choose to mail your application (be sure and fill the form out as required before printing it) you also have the option of paying by check instead of credit card. If so, simply leave the credit card info blank on the form.
